RANK ONE ELECTRONIC PARTICIPATION FORMS

These forms are required by the UIL and Ennis ISD to maintain eligibility throughout the year.  Without each of these forms on file, a student athlete may not be permitted to participate in any UIL athletic event.

Scroll over the heading "Electronic Participation Forms", select one of the ten forms from the drop down menu.  Repeat following steps for all 10 forms.

 

1) Complete all required fields 

2) Sign in signature boxes by clicking your mouse and dragging it across the screen as if it were a pen on paper. 

3) Check the box that says "I agree" next to your signature.

4) Enter a valid email and hit submit.

 

All forms will be immediately electronically submitted to our EISD account where they will be either approved or declined based on your adherence to the steps listed above.  Rank One Sport will notify you via your submitted email as to whether the form was accepted or declined.
